Subsequently, our coordination department will promptly begin, as what we endearingly refer to as, the “Fit for You” process. During this process, our coordinators will refer to the nurse’s notes collected during the initial assessment and will select the most suitable and compatible caregiver based on your specific requests and needs. Your assigned coordinator will follow up with you in a timely manner to begin services once this process is complete.

From there, you will have access to our selection of other comprehensive services which include certified home health aides, live-ins, skilled therapies, medical social work, daily wellness calls, and more! Steps to Care

1. INITIAL CALL: Our intake team will gather your information over a brief 5-10 minute phone call in order to set up an in-home visit.
   
2. IN-HOME CARE ASSESSMENT: Here we will discuss your questions and concerns related to your care. A customized Plan of Care will be created to be suit your individualized needs.
   
3. START OF CARE: We will come to initiate services with the caregiver and review your Plan of Care as well as orient the Caregiver to your particular case to ensure that all your goals are being met.
   
4. CLIENT VISITS: Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Plan of Care.
